Appium - Android Real Deivce - New session could not be created - Device not online

Hi - I am facing this issue. Whenever I try to execute my test using Appium and Android - Samsung Galaxy real device. I am seeing the chrome browser being opened but closed immediately. This device is listed in adb devices too. Appium version: 1.4.16.1 ; Android 5.1.1. Tried shutting off and on USB debugging but to no use.
Below is the Appium log, Any help will be much appreciated. Thanks!

What’s listed when you run adb devices ?

If you dont find any devices, follow the steps listed in http://www.dominantwire.com/2012/11/how-to-solve-adb-device-not-found-error.html

As per the logs, Appium is unable to find devices online ([Error: An unknown server-side error occurred while processing the command. (Original error: unknown error: Device ddf0b6db is not online)

1.check our adb devices
command -adb device
2.kill and restart our server
command -adb kill-server

Running adb devices in cmd displays ddf0b6db as a device
ddf0b6db device

Killed and restarted server. I am seeing browser is opened in mobile(real device), but closed immediately with the same error.

Can u please send screenshots

Actually when I kill and restart server and run the test script, I am seeing browser is opened, but closed immediately. But the error in eclipse is:

from unknown error: Runtime.executionContextCreated has invalid ‘context’: {“auxData”:{“frameId”:“9726.1”,“isDefault”:true},“id”:1,“name”:”",“origin”:"://"}" and replacing the chrome driver are related.
